INFORMATION CLAUSE

for persons using the contact form on the website of the Creative Center for Education and Economic Cooperation in Hrubieszow

Pursuant to Article 13 (1) and (2) of the Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of personal data of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (Official Journal of the EU L 119 of 4.5.2016) – hereinafter RODO, we inform you that:

The administrator of your personal data is the Creative Center for Education and Economic Cooperation, located at 15-17 Rynek Sutki Street, 22-500 Hrubieszow, tel: (84) 535 35 02, e-mail: sekretariat@kceiwg.pl.
In matters concerning personal data, the Administrator’s designated Data Protection Officer can be contacted by e-mail at: inspektor@kceiwg.pl or in writing by directing traditional correspondence to the Administrator’s address listed above.
Your personal data in the form of name, e-mail address are processed on the basis of consent (Article 6(1)(a) RODO), which you have the right to withdraw at any time. Provision of personal and contact data is voluntary, but necessary to provide and send a response.
Withdrawal of consent does not affect the lawfulness of processing carried out on the basis of consent before its withdrawal.
The processing of the personal data provided in the content of the contact form is carried out for the purpose of communication and answering the question asked by you.
Your personal data may be disclosed only to entities that are entitled to receive them under the provisions of the law, or transferred to entities that process personal data on behalf of ADO, on the basis of an agreement on entrustment of personal data processing.
Your personal data will not be transferred to countries outside the European Economic Area, which consists of the countries of the European Union and Iceland, Norway and Liechtenstein.
Your personal data will be stored until you object or the purposes of the processing cease.
You have the right to request from ADO access to your personal data, rectification (amendment) of your personal data, and the right to request erasure or restriction of processing, as well as the right to request erasure or restriction of processing, also to object to processing, if further processing is not necessary for ADO to comply with a legal obligation and there are no other overriding legal grounds for processing.
No automated decisions will be made against you (decisions without significant human involvement), personal data will not be subject to profiling.
You have the right to lodge a complaint to the supervisory authority – the President of the Office for Personal Data Protection, in the event that the processing of personal data violates the regulations on personal data protection.
